L ABORATORY S AFETY P ROGRAM

6.4.5 Facility Evaluation - Laboratory 6.4.5.1 Personnel working in laboratory areas conduct a brief evaluation of the area before work is done. This evaluation is intended to alert the employee to potential hazards present in the space.

T RAINING 7.1

Employee Training Requirements 7.1.1 Employees shall be trained in:

 Methods and observations that may be used to detect the presence or release of a hazardous chemical, such as: monitoring conducted by the customer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released.  The physical and health hazards of chemicals in the work areas. This must meet the requirements of the Hazard Communication Program  The measures that employees can take to protect themselves from hazards, including specific procedures the customer has implemented to protect personnel from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment.  The employees shall be trained on the applicable details of the customer’s written CHP.  The employees shall sign the training roster after receiving training on the Laboratory Safety Program. R ECORDKEEPING AND A CCESS TO R ECORDS 8.1 Training Records: 8.1.1 Copies of the training logs are maintained for the length of employment. Training records are stored in each employee’s training file. 8.1.1.1 Training records will include the following information:  Date of training session.

 Contents or summary of the training.  Name of person attending the training.  Names of persons conducting the training.

8.2 All employee records that contain measurements to monitor employee exposures, any medical consultation, and exams are kept in accordance with Medical Recordkeeping Program.
