L ABORATORY S AFETY P ROGRAM

 Capable of protecting our employees from health hazards associated with hazardous chemicals in the laboratory. Contact Corporate HSSE for assistance in determining the adequacy of the customer’s CHP.  Capable of keeping exposures below action levels or PELs.  Readily available to our employees for review.  Criteria that the company uses to determine and implement control measures to reduce employee exposure to hazardous chemicals including engineering controls, the use of PPE, and hygiene practices.  Provision that fume hoods and other protective equipment function properly and specific measures taken to ensure proper and adequate performance of such equipment. If employees are going to work on fume hoods then the customer must ensure that the equipment is made non-hazardous prior to the company conducting work. This verification may be documented utilizing a permit system to provide evidence that the hood is clean.  Provisions for pertinent information to be given to employees by the owner of the lab prior to our entry to the lab.  The circumstances under which a particular laboratory operation, procedure, or activity shall require approval from the customer before implementation of work.  Identification of areas requiring medical consultations and medical examinations.  Listing of personnel responsible for implementation of the CHP, which includes the assignment of a chemical hygiene officer or committee.  Information regarding any additional employee protection required for work with particular hazards. This includes select carcinogens and toxins.  Procedures for safe removal of contaminated waste.  Decontamination procedures for the laboratory being entered.  Annually, the Site Manager obtains any revisions the customer makes to the CHP.

6.2.3 Medical Consultation and Medical Examinations 6.2.3.1 The Site Manager provides all employees who work with hazardous chemicals an opportunity to receive medical attention, including any follow-up examinations which the examining physician determines to be necessary, under the following circumstances:
